Each parent must complete and submit their own application form prior to commencing the contact service.

Please download the Application Form via the link below and email your form along with any required supporting documents to info@reconnectingfamilies.au.

Submission checklist

• Completed application form (one per parent)

• Any relevant court documents (parenting orders, IVOs, ICL details, safety reports, interim/final supervised-contact orders)

• Email subject line includes your surname
